What Are All-On-4 and All-On-X Dental Implants?

All-On-4 and All-On-X are two ways to replace a full arch of failing or missing teeth with a fixed, permanent bridge. All-On-4 is a specific protocol using exactly four implants in a set pattern. All-On-X uses four, five, six, or more implants, chosen based on your bone and bite. Both give you a non-removable set of teeth, often the same day as surgery.

All-On-4 and All-On-X are both built for people who are done living with it. Each one replaces a full arch, top, bottom, or both, with a fixed bridge anchored to a small number of implants. The difference is in the approach: All-On-4 is a specific, well-tested protocol using exactly four implants, while All-On-X allows for a few more if your case calls for it. Either way, the result doesn’t slide, doesn’t click, and doesn’t come out at night.

I’ve practiced dentistry from my office in Midtown Manhattan for a long time, and full-arch cases are some of the most rewarding work I do. The change isn’t subtle. Patients eat differently. The process starts with a thorough evaluation. I take a full set of images and scans to see exactly what’s going on, your bone structure, your bite, what’s salvageable and what isn’t. We talk about what your mouth looks like now and what it could look like instead. No pressure, no scare tactics. Just an honest picture.

From there, we decide which approach fits your case. All-On-4’s four-implant pattern, often with the back two angled for extra support, works well for a lot of patients. Others have bone or bite considerations where the more flexible All-On-X approach, with five or six implant posts, gives a stronger foundation. Neither is the “upgrade” version of the other. It’s about what your jaw actually needs.

On surgery day, the implants are placed and a temporary fixed bridge is attached, so you’re not walking around without teeth while everything heals. Healing takes a few months. Once your implants have fully integrated, we move to your final bridge, custom made, comfortable, and built to last.

It’s a longer process than getting a filling. But for patients who’ve spent years frustrated with their teeth, or their dentures, it’s often the first time in a long while that their mouth isn’t working against them.

How I Approach Full-Arch Cases

I’m the only dentist in this practice, which means the person planning your case is the same person restoring it. For both All-On-4 and All-On-X, the implant surgery itself is typically performed by an oral surgeon I work closely with and trust completely. I oversee the planning, the choice between approaches, the design of your new teeth, and everything that happens before and after surgery, so your care stays coordinated from the first consultation to your final bridge.

I’m also conservative by nature, which might sound strange for a procedure this big. But conservative just means I don’t recommend full-arch treatment, or push you toward more implants than you need, unless it’s genuinely the right call. Sometimes patients come in expecting one approach and leave with the other. I’ll always tell you what I’d do if it were my mouth.

What’s the difference between All-On-4 and All-On-X?
All-On-4 is a specific, well-established protocol: a full arch supported by exactly four implants, often with the back two placed at an angle for extra strength. All-On-X is a broader approach using four, five, six, or more implants, depending on what your bone and bite actually need. Neither is automatically better. The right choice comes down to your anatomy, and we figure that out together.

Will I be without teeth during the process?
No. On the day your implants are placed, you go home with a temporary fixed bridge already attached. You won’t be sitting around with no teeth while you heal. The final, permanent bridge comes later, once everything has settled.

How long do these implants last?
The implants themselves are designed to last for decades, often a lifetime, with good care, whether you have four or six. The bridge on top may need replacement or adjustment over the years due to normal wear, but the foundation is built to be permanent.

Who actually places the implants, you or a surgeon?
For full-arch cases, I work with an oral surgeon I trust to place the implants, while I handle the planning, the choice between approaches, and your restoration. You get specialized surgical skill and continuous oversight from someone who knows your case from the start.

I’ve been told I need dentures. Could All-On-4 or All-On-X work for me instead?
